Create new Survey in UMedTech Platform a.k.a. SKY

In order to create a new survey in the UMedTech Platform (a.k.a SKY), you must be logged have Admin permissions.
  1. Content
  2. Add Content
  3. Webform
  4. Webform Type: Survey
  5. Repeat Webform: NO
  6. Save
Begin entering the content of your form/survey using the drag and drop fields on the right hand bar.

  1. First, add a Fieldset which will essentially your category for entering the various questions related to that category
  2. To require a field to be completed by the user, you will want to go to the tab "Validation" for each field/option.  Once you check the Required box, a red asterisk will appear after each question.
  3. If you are adding a question with checkboxes, i.e. Yes / No response, you will then add a Checkbox field and drag into the Fieldset
    1. Add title of the question under "Title" i.e. Pre-program requirements
    2. To enter the information for each checkbox, open the Checkbox field and click on the tab "options".  Enter the information on each line to indicate each checkbox 
  4. If you are adding information for the participant to then sign to acknowledge, you would first add the Text Field of the information and then:
    1. Add Markup into the Fieldset box
    2. Enter the details you wish to outline in the "Markup" field
  5. For including a blank text box for the participant to enter something such as First and Last Name:
    1. Use the Textfield option
  6. For a signature box, use the Markup option
    1. Enter this code into the Markup field
    2. In the drop down box "Text format", select the option "Full HTML"
  7. To enter a field for the user to be able to upload a document:
    1. Add "File" field and enter the name of the document to be uploaded i.e. PCP Clearance
    2. Under the Tab "Validation" ensure the following boxes are checked:
