Below is a list of Fitness Reimbursement Programs offered by Medical Insurance Providers and how to become a qualified location so that your members are able to submit to their Insurance for reimbursement of their membership.
Harvard Pilgrim
Harvard Pilgrim members can get up to $150 per subscriber contract, per calendar year for membership dues at a qualifying fitness center. No record of total visits to the fitness center needed. Because plans vary, members will want to make sure they're eligible for this fitness reimbursement program.
Qualified fitness facilities are added to the approved network when one of Harvard Pilgrims member’s
submit a request for reimbursement for a location that is not already listed as a qualified location. The
member would need to submit a fitness reimbursement via the paper process and HPHC would review
the facility upon receipt of the reimbursement request.
Anthem
Anthem members can get up to $200 per subscriber contract, per calendar year for membership dues at a
fitness center. You just need to exercise regularly, 48 times per year to be exact. Because plans vary, members will want to make sure they're eligible for this fitness reimbursement program.
Currently, Anthem does not require a location to go through any qualifications. Simply instruct your members to complete the attached form.
